Benefits Advice Form
A new procedure has been created to assist the county and district law libraries in advising the Law Society of Upper Canada of any changes (such as new hires, terminations, salary changes, changes to hours of work, etc.) to employment that affect benefits. A Benefit Advice Form must be completed by the employee (or by the administrator if applicable) and submitted to the Human Resources Department at the Law Society of Upper Canada. Any further benefit changes will not be accepted without the completed form(s) and signatures. Please note that each form requires the signature of the association president.
